ScottMed Consulting and Client Agreement

This Agreement Must Be Submitted Before The First Session!

This agreement is effective as of the date of submission by the client and receipt by ScottMed Consulting, LLC.

 

I agree the consultant will provide the client with the best advice possible to support the client in all aspects of the premedical preparation and application process.  The client agrees that appointments with the consultant do not guarantee acceptance into medical school.  

 

All materials and information submitted by the client will be held in strict confidence by the consultant.

 

All original works of authorship resulting from the consultant’s premedical advising work shall remain the sole and exclusive property of the consultant.  The client shall not have any ownership rights to this information or material.

 

Each of the parties acknowledges and accepts that the consultant's engagement is not exclusive and that it is permissible for the other party to offer similar services to or obtain them from others, so long as doing so does not violate this agreement. This agreement will be governed and interpreted in accordance with the laws of the State of Florida. 

 

No press release or other written or oral disclosure of any kind regarding the terms of this Agreement may be made by either party without the other party's prior written consent; however, consent for such disclosure shall be deemed given to the extent that such disclosure is necessary to comply with governmental regulations or a valid court order. The terms of this Agreement are confidential. 

 

Any true and correct copy of this agreement created by reliable means shall be treated as an original. And, submission of this form shall be treated as an original.
